Patient Alignment to ACOs/MA Plans

MCH has a variety of solutions that increase patient alignment and attribution to Accountable Care Organizations and Medicare Advantage plans for physicians. These solutions focus on prospective, retrospective, hierarchical, and risk-adjusted attribution methods in addition to patient alignment algorithms.

Annual Wellness Visits and/or Scheduling (AWV Subsequent)

These visits are mostly administrative and are higher cost and more time-consuming when performed in office rather than a white label virtual visit performed by MCH staff inclusive of a review of assessments, and medications through MCH Advanced Practice Providers.

Chronic Care Management (CCM)

CCM is a service to engage patients with 2 or more chronic conditions. MCH offers best-in-class CCM. MCH patient engagement rates are consistently and significantly above competing CCM and EHR/EMR patient engagement programs, tools, and services

Complex Case/Care Management

Complex Case/Care Management provides for the care of patients with complex issues such as mental health or substance abuse. Communication occurs with the patients’ other care providers in more complex situations requiring case management intervention.

Remote Patient Monitoring (RPM)

RPM services are valuable for high-risk patients with chronic conditions to remotely monitor, triage, and direct these patients to physicians before an emergent event becomes necessary.

Transitional Care Management (TCM)

Transitional Care Management services address the hand-off transitional period between the inpatient stay and alternative community settings. After a hospitalization or other inpatient facility stay (e.g., in a skilled nursing facility), the patient may be dealing with a medical crisis, new diagnosis, or change in medication therapy requiring this type of patient management and support.

MCH engages a patient-facing application that allows for other services to be scheduled for on-site visits such as various labs, EKG, ABI, Opioid Assessments, and more.
